   simple as that

   a hole in the table and a guy who be willing to lose an eye if anything went wrong

   Well the trick is actually done while the camera is too high to see the table. So before Joker slams the guy on the table, he just nocks over the pencil so no one gets injured. But I am sure there is someone willing to have a pencil in their head.
